We participate with most local insurance plans. Please bring your insurance card and photo id so that we can make a copy for your record. Anytime that you have a change in insurance, address, or phone number, be sure to notify the office receptionist so that the change can be entered into our system.
Payments (co-payments and outstanding balances) are due at the time of service. We accept major credit cards, such as Visa, Discover, Mastercard, and American Express. All balances are expected to be paid at the time of service
